  • What are your office hours?

    We are open Monday through Friday, from 9:00 am to 4:00 pm, with closures on most federal holidays. We typically schedule in-person visits through the 3:00 pm hour, with any additional appointments usually provided via telemedicine.   • What insurances does your office accept?

    We accept plans offered by most major insurance carriers, including Florida Blue, Aetna, Molina, Cigna, UnitedHealthcare, PHCS, Medicare, Doctors, and more. A detailed list can be found by clicking the button below.

  • Do you accept uninsured patients?

    Yes! We offer avenues by which uninsured patients can receive care at affordable prices, which can be found by reading our financial policy, which includes a financial assistance application that should be filled out and submitted before your visit if necessary.

  • Where can I park for my in-person visit?

    Our office is located on the west side of SW 117th Avenue, between SW 89th St and SW 90th St. There is free curbside parking along both streets, as well as a parking garage located on SW 89th St where you can park for a $5.00 fee. Because the street parking has limited availability, we suggest to patients who wish to park for free to arrive 15 to 20 minutes before their scheduled appointment time. Any parking lot associated with a different building should not be considered accessible.

  • What will I need for my first visit as a new patient?

    We collect demographic and financial information prior to visits for new patients, so we can hit the ground running and avoid any hiccups along the way. We encourage patients to use the integrated electronic forms directly from our EHR, Tebra, upon confirming their appointment with our front office staff.
